NLDS DISCLAIMER  RELEASE OF LIABILITY: In consideration of being allowed to participate in any way in the North Langley Diamond Sports related events and activities, I agree: 1)  There is a risk of injury from the events and activities provided by North Langley Diamond Sports, including the potential for permanent injury or death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; 2)  As the parent/guardian of the registered child I hereby give my approval for the child to participate in any and all of the activities/events provided by North Langley Diamond Sports. I assume all the risks and hazards incidental to the conduct of the events and activities, and transportation to and from the events and activities. 3)  If I observe any unusual significant hazard during my presence or participation, I will remove the child and/or myself and bring such to the attention of the nearest official. 4)  I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS North Langley Diamond Sports, its officers and directors, organizers, coaches, managers, umpires, sponsors, supervisors, and any other such person and/or all of them, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASES OR OTHERWISE. TERMS OF REGISTRATION: I acknowledge that registration is not complete until payment has been received.  A child is not eligible to play until all additional NLDS requirements have been completed, including but not limited to: the payment of any volunteer or uniform deposits, Softball BC fees and/or rep fees as determined by NLDS. I acknowledge that a charge of $25.00 will apply to all NSF cheques.  Registration Fees become non-refundable after the child’s first game is played.  A service fee of $25.00 will be deducted from any refund. Requests for a refund must be submitted via email to registrar@nlbaseball.ca or registrar@nlsoftball.ca I give permission to North Langley Diamond Sports to use photos of my minor child to promote NLDS activities and to post full name of my minor child for the purpose of announcing team rosters, and I acknowledge that if I wish to withdraw such consent that I must do so in writing to secretary@nldiamondsports.ca.   I will furnish upon request, a certified copy of the birth certificate of the named child. If I request that my child play for a particular coach or with a particular player, NLDS has no obligation to comply with any request that I may make. I certify that the information given in this registration is true and correct. I agree to complete forms 3584e and 3923e (criminal records disclosure forms) prior to the start of volunteer duties that involve working directly with children registered in North Langley Diamond Sports.  Volunteer duties that require Criminal Records Checks may include but are not limited to:  Coaches, Assistant Coaches, Team Parents, Executive Members and 3rd party Training Coaches. I acknowledge that by proceeding to register my child by electronic methods and means that I have read, fully understand and agree to the terms of this RELEASE OF LIABILITY and TERMS OF REGISTRATION. I acknowledge that I have given up substantial rights by agreeing to the RELEASE OF LIABILITY, and do so freely and voluntarily without any inducement.  I DISAGREE            I AGREE- GO TO STEP 2
